Care Manager II - New Castle, PA

Description:

Catholic Charities of the Diocese of Pittsburgh is searching for a Care Manager II at our New Castle, PA location.  This position performs assessment, care management, advocacy, and service coordination for clients receiving home- and community-based services.  Duties are performed in compliance with the PA Department of Aging (PDA)/Office of Long-Term Living (OLTL).  The candidate will be required to make home visits.  

Qualifications:

  • Bachelor's degree in social work or related field required
  • 2+ years of related experience preferred
  • Excellent written and oral communication skills, including interview and assessment skills
  • Knowledge of the aging process and geriatric issues preferred
  • The ability to interact contructively with community agencies and resources
  • Ability to make home visits
  • Valid driver's license and reliable vehicle
  • Act 33/34 and FBI clearances
