Challenges: Options In Aging - Long-Term Care

Challenges: Options in Aging offers four programs with the goal of assisting individuals to return to, or stay in their home as an option to nursing home placement.
Nursing Home Transition, PDA Waiver, Nursing Home Diversion and Family Caregiver Support are all programs that provide a wide range of services to individuals who need nursing home level of care, live in Lawrence County, are 60 years of age or older, and want to live in the community rather than a nursing home.
A Care Manager will be assigned to determine program eligibility and to work with individuals and families to design a plan of care to meet individual care needs.
Nursing Home Transition
This is a new program in Pennsylvania for older adults and people with disabilities who reside in nursing homes. It offers individuals the alternative to live in a home environment with the assistance of home and community based services.
The goal of Nursing Home Transition is to educate individuals and families about available living options and how to access services and support so that they are more comfortable and confident about their choices and preference for community living.
Support is available for those who wish to return to their homes or to live independently in an apartment or house. Although 24-hour care is not available, services available may include but are not limited to the following: advocacy, assistance in locating affordable housing, and information and referral to home and community-based services.
PDA Waiver
This program is funded through the medical assistance program. If determined financially eligible, an individual can receive in-home services at no cost.
Although 24-hour care is not available, services such as Personal Care, Home Health Care, Home Delivered Meals, Medical Supplies, Respite Care, and Personal Assistance Services are just a few of the potential services available.
After you and your Care Manager agree on a plan to meet your needs, the Care Manager will authorize appropriate services. You will then be able to choose your service provider from a list of approved agencies/companies.
Nursing Home Diversion
This program provides an option for those individuals who are not financially eligible for the PDA Waiver program. Individuals can still receive in-home services, but at a lower frequency than the PDA Waiver program.
Costs of services are based on income and a sliding scale fee.
Family Caregiver Support
Caregivers of individuals who are functionally or care dependent are provided caregiver education, benefits counseling, and/or financial reimbursement for approved services.
