answered and counseling on options available in health insurance.
Information and Referral

Staff answers your call and assists you
in finding appropriate services. They help clarify the need and
offer guidance in selecting alternatives.
Legal
Through an agreement with Neighborhood
Legal Services, limited legal services are available. A brief
statement and screening determines eligibility.
Outreach
Challenges staff goes into the community
to seek out individuals who are eligible for services.
Long Term Care Ombudsman

Residents of nursing homes, personal
care homes, or other long- term care facilities can get an ombudsman
advocate to help them in situations where quality of care or residents
rights are subject to question and should be investigated.
Transportation
Transportation is available for
individuals 65 and older who need to travel to a medical appointment
or to center nutritional services. Limited to 36 trips per month.
Pre-Admission Assessment
Individuals who are accessing public
funds are assessed prior to admission to a nursing facility to
determine appropriate level of care and discuss potential options
for care. Assessments are also required for those applying for
the Social Security Supplement (SSI) in a PCH or Dom Care.
