Long Term Care Services

Looking for an alternative to nursing home care?
Live at home… in comfort!

Challenges, Options in Aging offers four programs with the goal of assisting individuals to stay in their own home as an alternative to nursing home placement. The PDA Waiver, Bridge, Nursing Home Diversion and Family Caregiver Support are all programs that provide a wide range of services to individuals who are nursing home level of care, live in Lawrence County, are 60 years of age or older, and want to live in the community rather than a nursing home. A Care Manager will be assigned to determine program eligibility and to work with individuals and families to design a plan of care to meet individual care needs.

PDA Waiver
This fast-growing program is funded through the medical assistance program. Services such as Personal Care, Home Health Care, Home Delivered Meals, Nutritional Supplements, Medical Supplies and Equipment, Adult Day Care, Respite Care, Personal Emergency Response Systems, and Home Modifications are just some of the potential services available in this program. If determined financially eligible, there is no cost for services. After you and your care manager agree on a plan to meet your needs, the care manager will authorize appropriate services. You will then be able to choose your service provider from a list of approved agencies/companies.

Bridge
This program acts as a "bridge" for those individuals who meet both the medical eligibility for nursing home level of care and income eligibility, but are over the asset level for the PDA Waiver program. These consumers can receive the same amount and type of services as in the PDA Waiver program. The bridge program provides an affordable alternative to nursing home placement by providing 50% of the cost of services in the home.

Nursing Home Diversion
This program provides an option for those individuals who are not income eligible for the PDA Waiver or Bridge Program yet can still receive in-home services in their home. The same services are provided, but at a lower frequency than the Bridge or PDA Waiver program. Costs of services are dependent on income and are based on a sliding scale fee.

Family Caregiver Support
Caretakers of individuals who are functionally or care dependent are provided caregiver education, benefits counseling, and/or financial reimbursement for approved services.
